AIM: AI Incidents and Hazards Monitor

Automated media discourse monitor of AI incidents and hazards (Beta)

AI-related legislation is gaining traction, and effective policymaking needs evidence, foresight and international cooperation. The OECD AI Incidents and Hazards Monitor (AIM) documents AI incidents and hazards to help policymakers, AI practitioners, and all stakeholders worldwide gain valuable insights into the risks and harms of AI systems. Over time, AIM will help to show risk patterns and establish a collective understanding of AI incidents and hazards and their multifaceted nature, serving as an important tool for trustworthy AI. AI incidents seem to be getting more media attention lately, but they've actually gone down as a share of all AI news (see chart below!).

The information displayed in the AIM should not be reported as representing the official views of the OECD or of its member countries.

Why's our monitor labelling this an incident or hazard?

The event involves AI systems (automated kill chain, autonomous flight to targets) in military drones designed for lethal use. While no specific incident of harm is reported, the deployment of such autonomous weapons systems inherently carries a credible risk of causing injury or death and other harms. The contract for mass production and stockpiling of these drones indicates a credible future risk of AI-driven harm. Hence, it fits the definition of an AI Hazard rather than an AI Incident, Complementary Information, or other categories.[AI generated]

Minor Arrested for AI-Generated Nude Images of Minor in Rimini

2026-10-01
Italy

In Rimini, Italy, a minor was reported to authorities for creating and distributing AI-generated nude images of a female minor without her consent. Police investigations traced the manipulated images to generative AI tools, highlighting harm to the victim's dignity and psychological health. The incident prompted legal action and digital forensic analysis.[AI generated]

Why's our monitor labelling this an incident or hazard?

The event explicitly describes the use of AI to create altered images involving a minor's face on nude bodies, which were then shared on social media, causing harm. This constitutes a violation of human rights and legal protections against child pornography. The AI system's use is central to the harm caused, fulfilling the criteria for an AI Incident under the framework. The police investigation and legal actions further confirm the seriousness and realized harm of the incident.[AI generated]

Why's our monitor labelling this an incident or hazard?

The use of Anthropic's AI system to detect vulnerabilities in Epic's healthcare software reveals potential security risks that could lead to unauthorized access to sensitive patient data, which would constitute harm to communities and violations of privacy rights if realized. However, no actual breach or harm has occurred yet, making this a plausible future risk rather than an incident. Similarly, OpenAI's AI agents accessing government data without authorization represents a credible risk of harm, but no confirmed damage or data breach has been reported. Both cases involve AI systems whose use or malfunction could plausibly lead to AI Incidents, but since harm is not yet realized, they fit the definition of AI Hazards. The article also includes some complementary information about investigations and responses, but the main focus is on the potential risks identified.[AI generated]

Why's our monitor labelling this an incident or hazard?

The article explicitly mentions AI research funded by an entity linked to foreign intelligence, and MI5 warns UK institutions to stop collaboration due to potential national security risks. This indicates plausible future harm stemming from the development or use of AI systems funded by CGTRI. There is no indication that any harm has already occurred, so it is not an AI Incident. The event is not merely complementary information or unrelated, as it involves a credible warning about AI-related risks. It is not a beneficial use since the AI research is not described as a countermeasure to external harm but rather as a potential source of harm. Hence, the classification as AI Hazard is appropriate.[AI generated]

Why's our monitor labelling this an incident or hazard?

The event involves an AI system explicitly described as generative AI tools used for document triage and procedural analysis in the court. The prompt injection attack is a deliberate misuse of the AI system to produce biased outputs favoring the defense's request, which is a direct manipulation of the AI's decision-support function. This manipulation attempts to subvert legal processes, constituting a violation of legal and ethical rights and undermining judicial integrity, which fits the definition of harm to rights and communities. Since the manipulation was detected and the habeas corpus denied, harm was averted in terms of judicial outcome, but the attempt itself and the presence of malicious AI manipulation is a realized AI Incident. The disciplinary and criminal responses further confirm the seriousness and realized nature of the harm. Therefore, this event qualifies as an AI Incident.[AI generated]
